When removing pieces from the board, the two pieces must share at least one adjacent (empty) space; they can, but don't need to be, adjacent themselves. Place the next piece in a space that is adjacent to both of the to-be-removed pieces, then take the removed pieces from the board and return them to your stash. You can only do this if you have a piece of the appropriate size remaining, or if the removal of the pieces from the board gives you a piece of the appropriate size.
 
If the large is removed from the board in this process it may be replaced in an adjacent space; it may "move" in this fashion. Once it is on the board the large is considered "wild", and counts as a medium or a small for all purposes (both removal and four-in-a-row). The only way to get the large on the board is to remove two mediums.
